Anthony Stephen Chavez

Anthony Stephen Chavez, 42, passed away Tuesday, January 19, 2010 at Davis Hospital of cardiac arrest.

He was born August 12, 1967 in Salt Lake City to Raymond A. and Derinda J. Chavez.

Tony loved to eat, to give hugs, music and custom made helmets by Dave.

Thank you to all the staff who have loved and cared for Tony, guided by his guardian, Michelle.

A viewing will be held Tuesday, January 26th from 6 to 8 p.m. at Lindquist''s Roy Mortuary, 3333 West 5600 South.

There will be no formal funeral services.

Interment, Ogden City Cemetery.
