OGDEN a€" Dana William Herman Flackman, 45, passed away Wednesday, July 6, 2011.

He was born December 2, 1965 in Ogden, Utah the son of Richard Wayne Flackman Sr. and Donna Jean Park Flackman.

Dana married Shannon Marie Leonard on April 4, 1994.

He enjoyed the outdoors and was especially passionate about fishing. He loved to spend time with his children.

Surviving are his children: daughter Tiffany, son Morgan, twin daughters, Lindsey and Lacey, father Richard, step-mother Ellie, two sisters, Deborah (Jerry) Ivie, Decca (Don) Cunningham, brother, Richard Flackman Jr. and closest lifetime friends, Thomas Bird, David Alvarez Jr. and Kalani Mikeal.

Preceded in death by his mother, grandparents and nephew, Richard Wayne Flackman III.
