Son of Travis and Sarah Slade, brother to Andrew, Noah, Joshua, Leah, Gabriel, and Emma

Sam was an adventurer, a fun companion, a deep thinker, a pro gamer, and an excellent golfer. We love you, Donut!  And we miss you like crazy.

Sam’s Big Celebration will be held on Saturday, 04 March, 2023 at 10:00 am at the LDS Stake Center, 60 East Gordon Street in Layton, UT.  All are welcome.  No public viewing will be held.

Services entrusted to Lindquist’s Layton Mortuary, 1867 No. Fairfield Rd., Layton, Utah.
